Volunteer Application

CURRENTLY LOOKING FOR LOW-INCOME ASSISTANCE CASE MANAGERS. Some experience with dogs and cats is useful but not required. There is a learning curve, so interest in doing this for a fairly long period is preferred. It is necessary to have a computer since we use some management software (the software isn't difficult, but would not work well on a cellphone). We may ask applicants for references.

I understand this application is for performance of work as a volunteer and that such work will not be compensated. I am 18 years of age or older. Approval of this application is at the sole discretion of Animal Care Assistance Program (ACAP). If I begin work as a volunteer such voluntary services may be terminated by either party at any time. In the event of such termination by either party I will promptly return any equipment provided using a shipping label provided by ACAP.

Release and Waiver. I hereby release and forever discharge and hold harmless Animal Care Assistance Program (ACAP) and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, that arise or may hereafter arise from my activities with ACAP.
